Appointments

Patients wishing to make an appointment can directly ring our Metro Pain and Metro Spinal Clinics. (For telephone listings, please see our Locations). A written referral, required prior to your appointment, can be faxed, posted or emailed to the location where your consultation is taking place. It is essential to include your contact details. It is also important to let us know at the time of booking whether this consultation is related to a Workcover or TAC incident and to provide us with details of claim numbers, Insurer and Employer details.

Pre-Consultation

After you have made the appointment we will email you  a “pre-consultation” questionnaire that  you can complete on line . If you are unable to provide an email address then we can post to you and the completed questionnaire can be returned in the stamped envelope provided. When you present at the clinic and prior to the consultation you will be asked to fill out an “Effectiveness Audit Sheet” which will be used in your clinical care.

Billing Arrangements and Fees

Initial or Review Consultations

Metro Pain and Metro Spinal Clinics are private billing facilities. You are expected to pay for the initial or review consultation on the day . Eftpos with an  on-line Medicare rebate facility  is provided for your convenience.  If your consultation is related to a Workcover or TAC incident and you have provided the claim details then it will be forwarded to the relevant organisation

Procedures
